Sober & Sovereign Coaching Agreement

Terms & Conditions

This agreement is made between Jourdan Rystrom and Blaze Bell (“The Coaches”) and whomever is checking this box (“The Client”), for participation in Sober & Sovereign (“The Program”).

By checking this box and proceeding with enrollment into the Program, the Client agrees to the following:

SOUL AGREEMENT

• I know that Jourdan and Blaze are not here to “rescue” or “save” me, but rather hold EXPANSIVE AF space for me to BLAST into my power.
• I know that in making this investment I am entering into a vortex of MASSIVE expansion… and oh baby am I ready!
• It’s not required that I feel comfortable 24/7 to show up 100%. I DESIRE this. Deep in my SOUL. I am ALL IN! 💫⁣

COACHING COMMITMENT

I understand that coaching is for individuals who are emotionally and psychologically healthy who want to make changes and move forward in their lives. I agree that I am voluntarily participating out of my own free will and therefore I choose to take with me what resonates and leave the rest. I am ready to show up eager to learn and I commit to being 100% authentic in all my interactions.

COACHING SESSION PROCEDURES

Group Coaching sessions may be conducted via Zoom or in our Private Facebook Group. 

Multiple modalities may be used such as breakthrough coaching, tapping, hypnotherapy, EMDR, reiki energy healing, and other holistic modalities. Client agrees to take full responsibility for how they show up and agrees to come to the sessions with a coachable attitude and willingness to see things from a new and fresh perspective.

CONFIDENTIALITY

The group coaching relationship is built on trust. The Coaches agree to keep all conversations and information within the sacred group container private and confidential. No personal ideas, information or thoughts expressed will be shared with anyone except with the permission of the Client.

Due to the nature of the group container with other parties being involved, we cannot guarantee 100% confidentiality but the Coaches do intend to show up in creating a sacred space that feels safe to share sensitive topics freely and authentically.

Some calls will be recorded (i.e. the ones on Zoom) and will be reposted in the group for other members to watch the replay. If the Client is live on the call, the Client gives permission for their likeness to be shared in this way. If the Client does not want to be seen in the recordings, the Client has the option to turn off their camera during video calls.

PAYMENT OPTIONS & ACCESS

MONTHLY PAYMENT OPTION

Sober and Sovereign accepts PayPal, debit, or credit card payments in the link provided for monthly automatic payments of $97.00 USD.

BI-YEARLY & YEARLY PAYMENT OPTION

Sober and Sovereign also accepts PayPal, debit, or credit card payments in the link provided for upfront payments of either 6 months for $547 or 12 months for $997.

The 6-month membership payment will renew automatically every 6 months and the 12-month membership payment will renew automatically every 12 months, both at their respective discounted rates.

ACCESS TO GROUP & ENERGETICS

These payments (whether monthly, bi-yearly or yearly) provide the Client access to the private group, live calls, session recordings, and all features for the duration of the time paid for. It is an intentional investment to also be in the energy and proximity of The Coaches, the connections, and the vibe of the space.

CANCELLATION & REFUND POLICY

The Client has the option to cancel future recurring payments at any time by emailing [email protected] with the subject line “Cancellation Request”. Please note, the cancellation email must be received 5 or more days before the next payment date in order to be processed.

If the Client chooses to not show up to calls, be inactive in the group for any reason, or even leave the group, that is their choice, but in no way does it constitute a reimbursement or refund. There are no refunds, only a cancellation of membership going forward once requested via email. No refunds or partial refunds will be issued regardless of attendance in the program.

If the Client chooses to cancel their membership for any reason, the Client understands that they will be removed from the Facebook Group and will lose access to the privileges and resources inside The Program.

DISCLAIMERS

The Client understands that the role of the Coaches is not to prescribe or assess health levels; provide health care, medical or therapy services; or to diagnose, treat or cure any disease, condition or other physical or mental ailment of the human body. Rather, the Coaches are mentors and guides who have experience in Health and Wellness Coaching + Hypnotherapy to help clients reach their own health goals by helping them devise and implement positive, sustainable lifestyle changes.

The Client understands that the Coaches are not acting in the capacity of a doctor, licensed counselor, therapist, psychologist, dietician-nutritionist, or any other licensed or registered professional, and that any advice given by the Coaches is not meant to take the place of advice by these professionals. If the Client is under the care of a healthcare professional or currently uses prescription medications, the Client should discuss any dietary changes or potential supplement use with his or her doctor, and should not discontinue any prescription medications without first consulting his or her doctor.

The Client has chosen to work with the Coaches and understands that the information received should not be seen as medical advice and is not meant to take the place of seeing licensed health professionals.

The Client understands that the Coaches teach and use spiritual tools and practices to help coach the Client, and the Client is never expected nor forced into believing the Coach’s religious or spiritual beliefs in order to participate.

PERSONAL RESPONSIBILITY & RELEASE OF HEALTH CARE RELATED CLAIMS

The Client acknowledges that the Client takes full responsibility for the Client’s life and well-being, as well as the lives and well-being of the Client’s family and children (where applicable), and all decisions made during and after this program.

The Client releases the Coaches from any and all liability, damages, causes of action, allegations, suits, sums of money, claims and demands whatsoever, in law or equity, which the Client ever had, now has or will have in the future against the Coaches, arising from the Client’s past or future participation in, or otherwise with respect to, the Program, unless arising from the gross negligence of the Coach.

CONFIDENTIALITY BY LAW

The Coach will keep the Client’s information private, and will not share the Client’s information to any third party unless compelled to by law.

AGREEMENT OF TERMS IN ORDER TO PROCEED

The Client agrees that they have read the aforementioned Sober & Sovereign Coaching Agreement Terms & Conditions in its entirety.

In addition to checking the Terms & Conditions box upon enrolling, the Client’s initial request to join the private Facebook Group also acts as confirmation of their agreement and understanding to all the terms and conditions stated herein.

This document will also be attached via link in the Client’s payment confirmation email for easy reference.
